Downloads...

You can start your process here.

Before downloading and completing these forms please read the following:

Both the Compliance Documents and The Uniform Residential Mortgage Application are PDF files. In order to launch these files, you must have a version of Adobe Acrobat Reader. If you need a copy, click the below button.

Scale pages (page scaling)

Page scaling reduces, enlarges, or divides pages when you print. If your PDF file does not print at the size you expect (that is, the output is either larger or smaller than you anticipated), select Page Scaling in the Print dialog box. To select this setting, choose File > Print, and then select an option from the Page Scaling menu. For a complete description of each option, consult the section entitled "Printing Adobe PDF Documents" in Acrobat Help.

Print PDF with mixed page sizes (Windows only)

To print a PDF that contains mixed page sizes:

  1. Choose File > Print.
  2. Select Choose Paper Source By PDF Page Size.
  3. Click OK to print.

With the application and disclosures, you can fill out as much or as little of the application as you like. I will complete it with your help and the supporting documents you send me. Your credit report will supply the liability section information. But be sure to sign pages 1 and 4. It's a new form and people often miss at least one signature page.

If this is a pre-approval application and no property has been identified, please don't date. I will do it for you when you find a house. The application expires in 90 days and will have to be redone if it is dated. Finally, mail the application and disclosure forms to :

Fill out the Paper Application...

You should read everything on this page before filling out your application.

Here’s where you can download, print, and complete the paper disclosures and the application.

click here for Andrew Nathan's application package.

click here for the Disclosure Documents.

In the attachments included you’ll find a loan application and credit authorization forms to be completed, signed, and returned. If you have any questions while filling the application out, don’t hesitate to call me at 415-256-9130. You can also reach Cynthia at (415) 453-6100. You can fax to my efax at 415-373-9220, scan and email, or mail to the address below.

Please read the following carefully:

You can fill out the application on the computer then print and sign it or print it first and fill it out by hand. This is not an interactive PDF so you can’t save your work. Don’t exit from the application while you’re filling it out or you will lose your entries. If you like, just fill out the very basic fields (name, address and employment) and be sure you sign the application on the middle of page 4. If you have questions about any of the fields on the application leave them blank and we will take care of them for you. We may need to call you to ask the necessary questions.

For instance: We can complete your assets and liabilities from your bank statements and credit report.

If you are going out of town for more than a day or two any time during this process let us know now (next couple of months)

One more important thing. Do not open any new accounts or embark on any activity which will trigger a credit inquiry during this process. Lenders may check your credit again just before funding your loan. Also don’t move any large sums of money around or make large deposits without being able to paper trail and source those funds. This can create problems should we need to provide additional statements.

Please send back the following with your loan package:

  • 2008 & 2009 W-2s
  • Current pay check stubs covering thirty days
  • Last two months’ account statements for checking, savings, investment, mutual fund, retirement and/or IRA accounts. Please include all numbered pages, even if blank. These need to be actual statements not online statements which usually do not have your name or complete account numbers for security reasons. PDFs of these actual statements are usually available on your bank’s website.
  • Check for $19 made out to Credco to cover the cost of running your credit report.
  • Clear copy of valid picture ID. (Driver’s license, Passport, or National Identity Card, etc).
  • Two years federal tax returns if there is investment income, investment property or self employment income. K-1s and 1120s if you have partnership or corporate income.
  • If you have an equity line or 2nd mortgage you plan to keep - a copy of the note
  • Current mortgage statement for all mortgages.
  • Insurance declaration statement

Getting a loan has become a more intrusive and demanding process. I apologize in advance for this challenge and ask you for your patience and cooperation as I guide you through it’s confusing aspects.
